Silent Hill: Townfall relocates the horror franchise from its iconic town to St. Amelia, a Scottish island hamlet where protagonist Simon Ordell arrives after being mysteriously summoned. He encounters Zoe Ellis, a nurse with connections to his past, and Richard, a mysterious figure offering assistance. Like previous Silent Hill games, their fears and past trauma manifest as monsters and psychological horror.

The game takes a first-person perspective, a notable departure from the series' traditional third-person view. This design choice affects gameplay significantly. Half the game takes place in dark, claustrophobic environments where players must search small rooms for items, potentially causing motion sickness for sensitive players. However, the emphasis on stealth rather than direct combat makes the experience more accessible than expected.

Stealth forms the core survival mechanic. Simon's CRTV device displays enemy positions and directions, providing crucial information for sneaking past threats. Combat remains risky: enemies deal heavy damage, melee weapons break quickly, and guns attract unwanted attention. Shooting is a last resort reserved for large, predictable targets. Waiting for the right moment to advance can require significant patience, and sometimes no safe window exists. The game rewards smart play with generous item availability, allowing players to finish with excess resources if they manage their approach carefully.

Playing as different characters adds variety to the experience. Zoe Ellis sections shift gameplay dynamics: her pager lacks directional information about monsters, while enemies in her levels attack from range. This absence of specific information creates moments where monsters can strike unexpectedly. Despite some frustration with these sections, the reviewer found Zoe's narrative compelling and wanted more of her story, suggesting her character remains underutilized overall.

The game draws comparisons to Silent Hill 2 through its exploration of characters' otherworlds and trauma-focused psychological horror. While some longtime fans may question Konami's decision to move the setting away from the titular town, the new location allows exploration of different stories, aesthetics, and gameplay styles. The first-person perspective, initially jarring, integrates into the series through narrative depth and psychological themes.

Puzzles throughout the game require observation and pattern recognition. The pharmacy locker code, the community gardens shed combination, and the medical clinic scales represent the types of challenges players encounter. These puzzles integrate story elements and progression mechanics rather than existing in isolation.

Silent Hill: Townfall launches September 24, 2026 for PS5 and Windows PC.
